Virtually designed polycaprolactone meshes, 3D printed and combined with a xenogeneic bone substitute, were employed. A cone-beam computed tomography scan was obtained pre-operatively, immediately post-operatively, and 15 to 24 months following the insertion of implant prostheses. This report features a unique case that utilized a staged intervention strategy to address a problematic implant site which resulted in a delayed sinus graft infection, sinusitis, and an oroantral fistula. The interventions included functional endoscopic sinus surgery (FESS) and a press-fit block bone graft technique. Sixteen years ago, a 60-year-old female patient underwent a maxillary sinus augmentation (MSA), with three implants being simultaneously placed in her right atrophic maxillary ridge. The advanced peri-implantitis necessitated the removal of implants #3 and #4. The patient subsequently experienced a purulent drainage from the wound, a headache, and complained of air leakage due to an oroantral fistula (OAF). A mean follow-up period of 4831324 months revealed no statistically significant difference in the rate of MRS progression between participants categorized as DSM and non-DSM (P = 0.7462). In the DSM cohort, patients whose MRS condition worsened exhibited a greater age and higher refractive error compared to those with stable or improving MRS (P = 0.00301 and 0.00166, respectively).
